Please send them to our encrypted email ( or by secure fax 888-972-6908, please include a copy of your insurance card at least 3 days before your scheduled appointment. A 24 hour notice of cancellation is required for all appointments, to avoid a $50 late cancellation fee.

We currently contract with Regence,Bridgespan, Premera, Lifewise, and First Choice Health Network plans: we are out of network providers for all other insurance companies so please check the terms of your insurance beforehand.

We offer an affordable concierge plan- a real alternative to disease management, you can pay a membership fee (non-refundable) which allows you access to discounts on supplements, discounts on lab tests, including genetic test interpretation, phone consults, modalities that insurance companies may not cover (i.e. NAET, NLP/hypnotherapy, etc..) and extended appointments for a monthly base fee. This is not an alternative to health insurance, but an excellent way to ensure you get comprehensive responsive health care for an affordable price. If you have a high deductible Health Savings Account/Flex Account this is an excellent complement to ensure you get excellent personalized health care.
